5) The list of source certificate form O (O) issued by Fertilizer Supplier / Producer Company

6) Copy of business place details in the A4 size of the map

7) Copy of A4 size of the map, including the storage location details (upload the same file if the sale/business site and storage site is the same)

8) If the business site is a tenancy, then the copy of the tenancy contract

9) If the storage area is of tenancy, then copy of the tenancy contract
